If your insurance provider is not listed on this page, payment is due at the time the services are rendered, and you will be responsible for filing with your insurance company. Arangements can be made to ensure that your insurance company is billed and you are not responsible for the entire ammount at the time of visit. Please call our office at (812) 376-0166, for more information.


If you have dental insurance, our office will file your claim as a courtesy. It will be your responsibility to ensure that your insurance ultimately pays. This will allow you to only pay an estimated partial payment and then will be responsible for the remainder of what your insurance will not pay.
