Dental Provider for plans 100, 200, 300, and Specialist, and Guardian Insurance Provier. We also file all other Insurance
Claims and we accept all other available dental isnurance.

If you have dental insurance, our office will file your claim as a courtesy. It will be your responsibility
to ensure that your insurance ultimately pays.
